Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. Responsibilities
- Conduct client home visits
- Conduct comprehensive assessments to determine client needs and develop personalized service plans.
- Facilitate access to community resources such as healthcare, housing (including Section 8), and workforce development programs.
- Collaborate with multidisciplinary teams to ensure holistic care for clients, including social work and child welfare services.
- Monitor client progress and adjust service plans accordingly to ensure effective outcomes.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies.
- Utilize motivational interviewing techniques to empower clients in their journey towards self-sufficiency.
Experience
- A background in social work or a related field is preferred, with experience in case management or direct client services highly desirable.
- Strong interpersonal skills with the ability to build rapport with clients from various backgrounds.
- Excellent organizational skills and attention to detail are required for effective case management.
