The individual will be an integral member of pharmacy services directly impacting the utilization management of specialty injectables and high cost medications. Under the priorities of improving quality and driving value based prescribing, this individual will collaborate with cross functional colleagues to deliver value based care.

Primary Responsibilities:
- Works collaboratively, (using approved collaborative practice agreement) with primary care physicians, specialists, staff and leadership to develop and run programs that improve patient outcomes through appropriate medication management.
- Provides expertise and vision to Optum formerly known as Beaver Medical Group Medical Administration and leadership in regard to clinical and fiscal pharmacotherapy issues.
- Effectively utilizes and manages supporting ancillary staff such as pharmacy technicians.
- Maintains a high level of clinical knowledge of chronic disease states and changes in pharmacy practice in adult patient populations to serve as an authoritative information source on drugs and their utilization in therapy
- Assesses patients for medication adherence barriers and develop a plan to maximize patients’ medication success through improving or maintaining adherence to medications
- Develops and implements methods to measure the results of these programs to ensure that they meet quality and financial goals, as well as drive continuous quality improvement efforts.
- Delivers high quality evidence-based therapeutic recommendations to providers
- Delivers a patient-centered, clinically collaborative medication consultation
- Develops a post-consultation list of action items for the patient and/or prescriber
- Documents clinical consultations into internal data base: (EMR EPIC Systems)
- Provides coordinated, comprehensive, high-quality care to Optum formerly known as Beaver Medical Group patients as a key member of the patient care team.
- Interacts with other departments and present reports and findings to Optum, formerly known as Beaver Medical Group leadership as needed to perform the duties of this position.
- May be assigned special projects or other assignments and work tasks that are generally within the scope and level of the position, and relative to the need for flexible Company operations.
- Performs all other related duties and provides assistance to other patient clinics as assigned

Required Qualifications:
- Graduate from an accredited school of pharmacy
- California State Pharmacy License and Pharm.D
- 2 years of experience in clinical pharmacy practice including:
- Clinical pharmacotherapies
- Disease state management and drug use evaluations
- Reviewing coverage determinations and appeals
- Formulary and drug information resource
Preferred Qualifications:
- Advanced Practice Pharmacist (Aph) License
- Board certification in pharmacy specialties
- Managed care experience
- ASHP-accredited residency in Pharmacy Practice or equivalent background/experience

Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
