Case Manager: Community Support Program
Located in Dayville, Connecticut, 30 minutes from Providence, RI – Worcester, MA – New London, CT.
Description:
As a Case Manager, you will have the opportunity to forge a therapeutic connection with an adult diagnosed with a mental health disorder. Individuals often attribute positive changes in their lives to having someone who gave them hope for a safe and decent housing, access to quality mental health and medical care, meaningful personal relationships, productive activities, and a connection to their community.
The Community Support Program is a multidisciplinary team providing case management, housing services, individual support, and group counseling. If you have experience in mental health, substance use, community support services, and understand psychosocial issues of this population, please consider joining our team.

We are a dedicated staff of over 250 professionals, proud to continue growing in our mission of "Creating Healthy Communities" by providing top-notch, caring mental health and social services to children, adults, families, and communities of Northeastern Connecticut.
We offer a range of programs, including psychiatry, clinical psychology, prevention services, wellness, and employee assistance programs. Our offices are located in small towns and rural settings throughout Connecticut's "Quiet Corner," within easy commuting distance from New London, CT, Worcester, MA, and Providence, RI.
