Latino Outreach Peer Specialist
Location: Mansfield Center, CT
Position Overview
We are seeking a Latino Outreach Peer Specialist to join our dynamic team at United Services, Inc. This role is pivotal in enhancing our community engagement efforts with individuals who are Spanish-speaking and in need of substance use or mental health treatment. As a part of the Clinical Division, the Latino Outreach Peer Specialist will play a key role in promoting our services and fostering relationships with community partners.
You will have a unique opportunity to work with adults with substance use issues by connecting them with services. When individuals are asked what made the difference in their lives, the most frequent response is that they had someone they worked with who gave them hope that they can have what everyone wants: safe and decent housing, access to quality mental health and medical care, meaningful personal relationships, productive activities including work, and a connection to the community in which they live. Use of own vehicle and reimbursement at GSA rate. Bilingual/Spanish-English fluency required.
Job description
- Conduct outreach providing education on the resources in the area, connecting to these resources and provide direct hand off to appropriate services within the community or internally.
- Make contact by phone, video or in person with local community centers, Latino communities, congregate care agencies, housing areas and any other areas of need in the community.
- Develop and maintain required relations with staff and other service programs for the purpose of obtaining services.
- Assure connections to the agreed upon services, including monitoring and follow through to assure appropriateness and continuity of the services.
- Provide individual support, education, transportation, translation services and daily living skill development services to assigned clients to enhance client's independent functioning.
- Provide advocacy and ensuring continuity of care for assigned clients.
- Provide accurate, appropriate and complete records, documentation and reports.
- Perform other duties as assigned.
Schedule
Monday through Friday 9:00 A.M. - 5:30 P.M.
Qualifications
Associates Degree or High School Diploma/equivalent plus two years of relevant experience.
Salary Range:
$17.27 – $19.54 per hour, depending on experience
