Description:
United Services is seeking passionate and dedicated Case Managers to join our COAST (Community Outreach Assistance Support Team) program. In this role, you will support individuals living with mental health and substance use disorders who benefit from frequent, community-based support to maintain stability and continue their recovery journey. Working collaboratively with providers, community resources, and each individual on your caseload, you will help people access the services and supports they need to thrive.

What You Will Do (Day-to Day):
- Provide intensive case management services to adults with mental health and substance use disorders.
- Build trusting, relationships that promote recovery and personal growth.
- Coordinate care with healthcare providers, community agencies, and support networks.
- Conduct regular community-based visits and maintain frequent contact with individuals on your caseload.
- Assist individuals in accessing housing, healthcare, employment opportunities, benefits, and other community resources.
- Collaborate with individuals to establish goals and develop plans that support their recovery and wellness.
- Advocate for individuals and help remove barriers to achieving greater independence and stability.
- Document services and maintain accurate records in accordance with program and regulatory requirements.
Schedule:
Monday – Friday 8:00 a.m. – 4:30 p.m. with flexibility, no evening or weekends
Minimum Requirements:
Associate’s Degree in relevant human services field plus two years of relevant experience or Bachelor’s Degree in relevant human services field.
Salary/Hourly Range:
$19.00- $21.50 per hour depending on experience
Bilingual Spanish/English Fluency: $19.96- $22.59 per hour depending on experience

Work Location: Because our clients relay on face-to face support to build trust, this position requires employees to be fully present in the community and in the office.
