What You'll Do:
The LPN for Assisted Living plays a key role in providing high-quality resident care, medication administration, and clinical oversight in accordance with physician orders, facility policies, and state regulations.
- Administer medications and treatments, ensuring accuracy and compliance with physician orders and nursing standards
- Monitor resident health status, recognize changes in condition, and promptly report concerns to physicians and appropriate staff
- Document assessments, observations, and nursing care in the electronic medical record (EMR), including MAR/TAR entries and nursing notes
- Support resident care planning by participating in assessments, reassessments, and service plan updates with the interdisciplinary team and families
- Supervise and support Certified Nursing Assistants (CNAs) and Medication Aides while ensuring quality care delivery
- Ensure safe medication management, including narcotic counts, shift-to-shift accountability, and compliance with facility protocols
