Job Summary:
We are seeking a dedicated and compassionate Reliable Case Manager to join our homecare team. The Case Manager will play a crucial role in coordinating staff’s schedules, client's care, ensuring Client's schedules, planning doctors appt. and other clients schedule needs, and providing support to clients and their families throughout their Homecare journey. This position requires a strong understanding of behavioral health, home care, and being able to follow direction, but also give director and be able to delegate day to day tasks as they arise, and accept constructive criticism as needed ; this position may have an Assistant Case Manager that would follow direct instruction from this Position (Case Manager.) - depending on the case load at the time.
The Company Supervisor is at the company Monday - Friday 9a/10a-5p for support and training needs.
Position Summary:
The Case Manager plays a vital role in overseeing client care and ensuring that services are delivered according to each client’s individual needs, goals, and service plans. This position requires strong communication skills, attention to detail, and the ability to work collaboratively with clients, families, caregivers, and partnering agencies.
Key Responsibilities:
- Serve as the primary point of contact for assigned clients and caregivers.
- Conduct initial assessments, home visits, and ongoing monitoring to ensure quality of care.
- Develop, implement, and update individualized service plans in coordination with clients and families.
- Coordinate caregiver assignments based on client needs, preferences, and compatibility.
- Maintain timely and accurate documentation in accordance with agency and state regulations.
- Monitor service delivery to ensure compliance with client goals, policies, and funding requirements (e.g., Regional Center, Long Term Care Insurance, or private pay).
- Identify and address issues related to safety, performance, or client satisfaction.
- Provide ongoing support, coaching, and communication to caregivers.
- Participate in interdisciplinary meetings, case reviews, and audits as needed.
- Report and follow up on incidents, changes in condition, or concerns promptly.
Qualifications:
- Associate’s or Bachelor’s degree in Social Services, Human Services, Psychology, or related field is helpful and preferred.
- Minimum of 1–2 years of experience in case management, case/care coordination, or home care preferred.
- Knowledge of in-home care programs, Regional Center services, or Medi-Cal funded supports a plus.
- Strong communication, problem-solving, and organizational skills.
- Proficient with Microsoft Office Suite, EFAX and other electronic documentation systems.
- Ability to work independently while managing multiple priorities.
- Reliable transportation and willingness to travel locally for home visits.
Preferred Skills
- Experience working with adults or children with developmental or physical disabilities.
- Bilingual (English/Spanish) preferred.
- Familiarity with HIPAA and confidentiality requirements.

- Monday – Friday schedule (some flexibility required).

Qualifications:
- at least one year of office Leadership/ or management Position.
- At least, some caregiving experience. (Preferred)
- High School Diploma or GED
- College Experience (Preferred)
- Ability to obtain and maintain California Home Care Aide Registration
- CPR/First Aide Certified
- Clear TB Test
- CNA or HHA experience preferred
- Valid Driver License, Auto Insurance, and Reliable Auto
