The Hospice and Palliative Pediatric Physician is responsible for oversight of the medical component of hospice and palliative care to our pediatric patients in conjunction with the Medical Director. Provides clinical support and education to the hospice and palliative multi-disciplinary team. Collaborates with internal and external providers and referral sources to expand access to services. Hospice and Palliative Pediatric Physician will work within an interdisciplinary team to provide consultation to patients with serious illness who are seeking assistance with advanced symptom management, clarifying goals of care, addressing end of life issues and care, and transition planning.

Essential Functions

  • Determines patient eligibility for pediatric hospice and palliative service in accordance with program acceptance criteria and the Conditions of Participation.
  • Certifies and re-certifies patient eligibility for the pediatric hospice and palliative program in conjunction with the attending physician on a timely basis.
  • Provides clinical support to the hospice and palliative multi-disciplinary team.
  • Consults with primary physicians or specialists as needed regarding patient's medical condition. Assume responsibility for the general medical needs of pediatric hospice and palliative patients to the extent that the attending physician does not meet the needs, serving as attending physician as needed and when requested by patient or their representative. Work with the attending physician to encourage their fulfilling of their responsibilities.
  • Serves as a liaison between the attending physician and pediatric hospice and palliative team on an ongoing basis.
  • Evaluates patient's medical status in the home, outpatient and inpatient settings based on a visit schedule.
  • Works within the pediatric hospice and palliative provider leadership team to ensure adequate delivery of care. Serves on the clinical leadership team.
  • Works with the interdisciplinary team in establishing, implementing, and reviewing the pediatric hospice and palliative plan of care for each patient in keeping with the conditions of participation.
  • Attends patient care conferences as requested.
  • Develops/reviews and evaluates medical standards, policies and procedures for the pediatric hospice and palliative program and agency as requested.
  • Develops and provides in-services for the hospice team as requested by supervisor or agency leadership.
  • May be asked to provide ongoing community and professional education and network with hospital and medical practice staff to create referral sources and provide educational seminars on pediatric hospice and palliative care.
  • Performs home visits and visits for inpatient pediatric hospice and palliative services related to patient assessment and care as needed and appropriate.
  • Work directly with the patient, families, caregivers, and their hospital and specialty providers to create a customized plan of care for home-based pediatric hospice and palliative care.
  • Assists the interdisciplinary team (IDT) in developing and implementing a plan of care that is coordinated with the attending physician.
  • May be asked to provide coverage and support to pediatric hospice and palliative staff for the provision of medical services in the absence of the attending physician or after normal business hours.
  • Renders the necessary medical management, in consultation with the attending physician, consistent with the plan of care and the patient's needs.
  • Acts as a resource to pediatric hospice and palliative staff, patients, family members, and attending physicians regarding pain management and symptom management control.
  • May be asked to coordinate networking with community physicians to promote pediatric hospice and palliative home-based program.
  • May be asked to assist with the promotion of the pediatric hospice and palliative care program with the business development department.

Requirements

  • Ideal candidate will have completed a Palliative Medicine Fellowship Program. BE/BC in Palliative Care.
  • All applicants must possess a full range of clinical skills and an interest in working with an innovative pediatric hospice and palliative practice.
  • Experience working in an interdisciplinary team delivering pediatric hospice and palliative medicine.
  • All applicants must have an unrestricted New York State Medical License and a federally issued DEA license.
  • Board-certified in pediatrics or family medicine.
  • One to two years of medical physician experience, including pain and symptom management experience
