Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to trach patients in their homes, ensuring their comfort and well-being. The ideal candidate will possess strong clinical skills, a commitment to patient-centered care, and the ability to work independently while collaborating with a multidisciplinary team. Thia will be an 8hour shift of 3-4days a week.
Duties
- Conduct comprehensive assessments of patients’ health status, including physical, emotional, and social needs.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
- Administer medications and treatments as prescribed while monitoring for effectiveness and potential side effects.
- Provide education and support to patients and their families regarding health conditions, treatment plans, and self-care strategies.
- Perform wound care, catheterization, and other nursing procedures as necessary.
- Ensure adherence to infection control protocols and maintain a safe environment for patients.
- Document all patient interactions accurately in the electronic health record (Epic) to ensure continuity of care.
- Participate in discharge planning to facilitate smooth transitions from hospital to home or other care settings.
- Collaborate with primary care providers and specialists to coordinate comprehensive patient care.
Experience
- Current RN licensure in the state of practice.
- Previous experience in home health, geriatrics, or travel nursing is preferred.
- Familiarity with infection control practices is essential.
- Experience in primary care settings or ambulatory surgery centers is a plus.
- Knowledge of discharge planning processes is beneficial.
- Skills in infant care or emergency room (ER) settings are advantageous but not required.
- Proficiency in using alora plus is a plus for documentation is a plus.

Pay: $75,000.00 - $85,000.00 per year
