LPN Responsibilities Include:
- Provide skilled nursing care to residents
- Monitor resident conditions and communicate changes to the interdisciplinary care team
- Administer medications and treatments in accordance with physician orders
- Maintain accurate and timely documentation
- Support residents and families with compassion and professionalism
- Collaborate with CNAs, nursing staff, physicians, and other departments to ensure quality care
Qualifications:
- Current New York State LPN License
- Prior LPN experience in a skilled nursing facility, rehabilitation or similar setting preferred
- Strong clinical assessment and critical thinking skills; compassionate, patient-centered approach to care
- Excellent verbal and written communication skills; detail-oriented with the ability to recognize and respond to changes in condition
- Team-focused mindset with a commitment to collaboration and quality care
- Fully vaccinated against COVID-19
