Position Overview

The Social Determinant Coordinator (SDOH) provides centralized service coordination for the Care Coordination Team surrounding the social determinants of care, including but not limited to, DME, Housing, Food Insecurity, Transportation, OPWDD, School District/Early Intervention, establishing care. This position is an entry-level position and serves as a key member of the care team coordinating care and supporting the social needs of the population. Incumbents may be cross trained in one or all three functional areas of the social determinant coordinator role (Food, Housing, and Transportation). Incumbents may only be responsible for one functional area or may cover more than one functional area of the coordinator roles.

Education/Experience

Minimum Required:

• High School Diploma or equivalent.
• Two (2) years of experience collaborating as part of a care team to coordinate community resources to address social determinants of health.

Preferred:

• Customer Service experience.
• Previous experience with direct patient care.
