Key Responsibilities
- Conduct clinical intake assessments to determine home healthcare or hospice needs, with a special focus on EEOICPA-qualified patients
- Educate patients and families on services available under EEOICPA, including covered conditions, DOL benefits, and documentation requirements
- Coordinate with internal teams and Department of Labor case managers to streamline the enrollment process
- Clearly explain care plans, insurance coverage, agency policies, and patient rights
- Ensure timely initiation of services and referrals to appropriate clinicians and specialists
- Maintain accurate and compliant documentation per EEOICPA and CMS guidelines
- Address patient and family concerns with professionalism, empathy, and problem-solving skills
- Stay up to date on evolving EEOICPA regulations, benefits, and healthcare policies affecting eligible patients
- Active and unrestricted Registered Nurse (RN) license in the state of Tennessee
- Experience in home health, hospice, or clinical intake, preferably with knowledge of EEOICPA or DOL-related programs
- Strong communication, organizational, and interpersonal skills
- Proficient with electronic health records (EHR) and basic office software
- Must pass a background check and drug screening
- Compassionate and patient-focused mindset, especially when working with aging or chronically ill populations
