The Service Coordinator’s responsibilities are:
- Development of the person-centered service plan that address Participant identified areas of need, goals, intervention, and progress of goals.
- Coordinate services for Participants in relation to their identified needs.
- Monitor authorized services through face-to-face in-home visits, phone calls, person centered planning team meetings, and contact with service providers.
The Service Coordinator position is performed remotely from home and will require intermittent travel within Bucks, Chester, Delaware, Montgomery and Philadelphia counties.

Position Requirements:
Must have a Bachelor's degree in social work, psychology or other related fields with practicum experience.
OR
In lieu of a Bachelor’s degree, must have at least three (3) years’ experience in a social service or healthcare related setting.
