The Service Coordinator’s responsibilities are:
- Development of the person centered service plan that address Participant identified areas of need, goals, intervention, and progress of goals
- Coordinate services for Participants in relation to their identified needs
- Monitor authorized services through phone calls, face-to-face visits, person centered planning team meetings, and contact with providers
The Service Coordinator position can be performed remotely and will require travel within Dauphin County.
Position Requirements:
- Must have a Bachelor's degree in social work, human services, psychology or other related fields with practicum experience, or in lieu of a Bachelor's degree have at least three(3) or more years experience in a social service or healthcare related setting.
