Qualifications:

Under the direction of the Clinical Manager - Specialty Services, Case Manager - Compass will coordinate with the Department of Children and Families (DCF) to provide family-focused, community-based, case management, and intervention services to high-risk youth at risk for being removed from their place of residence or separated from their families.

MAJOR RESPONSIBILITIES:

COORDINATION OF SERVICES: Provide collaboration with DCF, place of education and/or employment, as well as other resources available to the youth. When needed, help to connect the youth to primary care and other medical services. Asses the needs of the family system and refer family to additional services when appropriate. Assist youth in accessing any additional resources that may be available to them.

DIRECT SERVICE: Provide intensive (on average 3 hours a week per youth) case management, and crisis intervention services to clients and families primarily in the home. Work with the client and family to develop the skills needed to keep in the family unit intact. Provide case management services to the client including assessment, monitoring, and advocacy. Participate in the planning and implementation of division specific summer camps/services. Attend any school meetings, family safety planning meetings, CPS, or other meetings deemed necessary. Participation in the annual 5 week long summer Camp Be A Kid program as program staff.


NETWORKING: Communicate appropriate information to other staff, Department of Children and Families, families, guardians, and others. Serve as an advocate and/or liaison on behalf of the client, program, and/or agency with other community agencies to best meet the needs of the client.

SUPERVISION AND TRAINING: Assists in facilitation of all treatment team meetings to promote effective communication; develops goals following identification of client's and family's needs and oversees implementation of individual treatment plan. Maintain interagency, professional and parent/guardian communications, as well as communication between treatment team members in order to assure effectiveness of services. Attends workshops, conventions, meetings, etc., as agreed upon with supervisor.

ADMINISTRATIVE: Meets weekly with supervisor and submits monthly and quarterly program reports. Meet with the local DCF Director monthly to discuss cases and caseload. Follow UCS recordkeeping requirements for all cases. Treatment plans need to be updated every 6 months. Completes time sheets and other paperwork in a timely manner. All documentation should adhere to Medicaid standards.

*In addition to the above, performs other duties as assigned

REQUIRED QUALIFICATIONS

  • High School diploma required. Bachelor’s degree in Psychology, Social Work, or related field preferred.
  • Experience in case management, advocacy, outreach, assessment, crisis intervention, and supportive counseling necessary as well as strong ability to communicate clearly.
  • Valid Driver’s License
