Seeking dedicated Licensed Practical Nurse's for Full Time, Part Time, or Per Diem shifts at our 31-bed intermediate care facility in Commack.
Schedule: 7:00am-3:30pm, 3:00pm-11:30pm, or 11:00pm-7:30am
** All F/T and P/T LPN shifts at this location require at least one weekend day per week **
General Purpose of the Job Under the supervision of a Registered Nurse, assist in the overall health and safety of the individuals served.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Knowledge of all medications for all individuals.
- Administer medications and treatments as prescribed by physician.
- Generate incident, seizure, and illness reports.
- Monitor and report health care on daily flow report records.
- Instruct mid-level supervisors and direct care staff in above, i.e., hygiene, diet, weights, etc.
- Submit written monthly, quarterly, annual nursing/medical progress notes.
- Coordinate laboratory care and medical appointments (routine, fees, special and emergency).
- Maintain functioning interpersonal relationships with other disciplines.
- Maintain ongoing communication with other programs, i.e., client health care.
- Monitor program participants during mealtimes.
- Order, record, and maintain sufficient/adequate supply of medication.
- Maintain sufficient/adequate amount of medical supplies/materials.
- Maintain accurate medical records including medication administration sheets, Kardex, Formulary, etc.
- Provide emergency first aid. Coordinate emergency services.
- Supervise direct care staff in absence of mid-level supervisor.
Education and/or Experience
Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience.
