Key Responsibilities :

• Patient Assessment :  Perform comprehensive evaluations of patients’ speech, language, cognitive-communication, and swallowing abilities.
• Treatment Planning :  Develop individualized treatment plans based on assessment findings and patients’ needs, with a focus on achieving measurable, functional goals.
• Intervention and Therapy :  Provide therapy interventions for speech, language, voice, fluency, cognitive-communication, and swallowing disorders. Utilize evidence-based approaches tailored to the SNF environment.
• Collaboration :  Work closely with an interdisciplinary team, including physical therapists, occupational therapists, nurses, and physicians, to ensure holistic and coordinated patient care.
• Documentation :  Accurately document patient progress, treatment interventions, and outcomes in accordance with regulatory standards and facility policies.
• Family and Caregiver Training :  Educate patients, families, and caregivers on techniques and strategies to support communication and swallowing function outside of therapy sessions.
• Compliance : Ensure all treatment and documentation adhere to Medicare, Medicaid, and facility guidelines.
