Case Manager/Social Worker (K–12)
Responsibilities Foster a supportive, inclusive, and trauma-informed school environment where all students feel seen, valued, and safe. Build trusting relationships with students and families grounded in empathy, respect, and a shared commitment to each student’s well-being and success.
Provide case management and counseling services to students across grade levels per IEPs or I&RS referrals. Conduct individual and small group counseling sessions focused on social-emotional skill-building, conflict resolution, and self-regulation.
Collaborate with teachers, school leaders, and support staff to identify students in need of social-emotional or mental health supports and coordinate interventions that remove barriers to learning and promote student achievement.
Develop, implement, and monitor intervention plans that address students’ academic, behavioral, and emotional needs. Maintain accurate and confidential records of services provided, progress updates, and follow-up actions.
Serve as a liaison between school, home, and community agencies to ensure that students and families have access to appropriate resources, including mental health services, crisis intervention, housing, food security, and healthcare.
Participate actively in the school’s I&RS and Child Study Team processes, contributing to student evaluations, intervention strategies, and IEP/504 meetings as appropriate.
Support family engagement by maintaining open and consistent communication with parents and guardians, facilitating family meetings, and organizing workshops that strengthen home-school partnerships and equip families to support their child’s growth.
Promote and model UC TEAMS values by contributing to a positive and restorative school culture that prioritizes student dignity, equity, and belonging.
Ensure compliance with all applicable confidentiality laws, ethical standards, and New Jersey Department of Education and Department of Children and Families regulations.
Arrive at school each day by 7:30 a.m. and leave no earlier than 3:45 p.m.
Qualifications
- Master’s degree in Social Work, Counseling, or a related field required
- Valid New Jersey School Social Worker certification or eligibility required
- LSW or LCSW licensure preferred
- Minimum of two years of experience providing school-based social work or counseling services preferred
- Strong understanding of child and adolescent development, trauma-informed practices, and community-based supports
- Excellent written and verbal communication skills
- Demonstrated ability to collaborate effectively with educators, families, and external agencies
- Strong organizational skills and attention to detail
- Culturally responsive, reflective, and committed to continuous professional growth
- Unwavering belief in the potential of all students and persistence in helping them overcome challenges to achieve excellence
