Location: Onsite Jacksonville, Florida

Employment Type: Full-Time

Shift: Monday – Friday 8am – 5pm EST

Pay Range: $18-$22

Position Summary: Responsible for placement, scheduling, and attendance confirmation of service for our clients. Main point of contact for both referral sources and providers

Key Responsibilities

• Placement and management of therapy, diagnostic and advanced services referrals according to parameters set forth by management to meet the client specific needs.
• Schedule patient with provider location, enter patient information into EMR system, send all necessary paperwork to the provider.
• Communicate with referral source the patient’s visit date, location, and time.
• Confirm patient attendance, provide update to referral source, along with any subsequent appointments.
• Update EMR system, collect records for electronic chart and send copies to referral source.

Required Qualifications High School Diploma or equivalent required Ability to type with minimal errors Strong proficiency in Microsoft Excel Excellent organizational, time-management, and multitasking skills Work Environment & Physical Requirements Frequent standing, walking, sitting, bending, and computer use Ability to occasionally lift up to 30–50 pounds
