The Medical Social Worker plays a crucial role in our patient care team, providing support to patients and their families to ensure they receive the best possible care during their hospice journey. Key Responsibilities:
- Conduct comprehensive psychosocial assessments for patients and their families to identify their unique needs and challenges.
- Develop and implement individualized care plans, in coordination with the interdisciplinary hospice team, to address the emotional, social, and practical aspects of care.
- Provide emotional support, counseling, and education to patients and families to help them cope with end-of-life issues.
- Advocate for patients and assist in connecting them with community resources, financial assistance, and other support services.
- Collaborate with the interdisciplinary team, including physicians, nurses, chaplains, and other professionals, to provide holistic care to patients.
- Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions.
- Participate in regular team meetings to discuss patient care and provide input on care plans.
- Master's degree in Social Work (MSW) from an accredited institution.
- State licensure as a Social Worker.
- Previous experience in a healthcare or hospice setting is preferred.
- Strong communication and interpersonal skills.
- Compassionate, empathetic, and dedicated to delivering the highest quality of care.
- Ability to work effectively as part of an interdisciplinary team.
