Responsibilities
- Provide comprehensive post-discharge follow-up care for patients transitioning from hospital to outpatient settings
- Perform patient assessments, including history, physical examination, and medication reconciliation Develop and implement individualized care plans in collaboration with physicians and care teams
- Coordinate care with primary care providers, specialists, and community resources Educate patients and families on treatment plans, medications, and self-care strategies
- Manage overflow same-day or acute care visits as needed
- Support initiatives to improve care transitions, patient outcomes, and reduce hospital read missions
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current North Carolina licensure (or eligible) as an NP or PA
- Minimum of 2 years of clinical experience required
- Experience in Emergency Department (ED) settings
- Experience in Primary Care
- Location: Goldsboro, NC area (specific site to be determined)
