The APRN will build strong working relationships with administrators and staff throughout their designated building and serve as the primary point of contact for emergencies and urgent care visits. May also provide care to assisted living facility residentsYou will verify admission orders and conduct initial assessment of residents. Conduct physical exams, medication review, and depression screeningYou will collaborate with facility Medical Director, attending physicians and nursing staff to deliver the highest level of care to the patientYou will communicate with and counsel family members on treatment options when there are material changes in statusResponsibilitiesProvide primary, urgent, and preventative care for residents in assigned skilled nursing facility. May also provide care to assisted living facility residents.Verify admission orders and conduct initial assessments of residents.
