Essential Functions and Responsibilities:
- Optimize quality of life for residents in skilled nursing, assisted living and/or long-term care settings working alongside an interdisciplinary team that includes medical professionals, psychotherapists and facility staff.
- Assessment, diagnosis, treatment and medical management of conditions including anxiety, depression, adjustment disorder, and pre-existing mental illnesses.
- Maintain timely, accurate documentation.
- Provide education to facility staff regarding psychiatric care and patient needs.
- Ensure compliance with CMS guidelines and facility policies.

Qualifications:
- Unrestricted PMHNP license.
