Job Overview
Ukiah Post Acute is a beautiful 52-bed facility in the heart of Ukiah, CA. We are seeking a compassionate and dedicated Licensed Nurse (LVN on RN) to join our healthcare team on the Night Shift. The ideal candidate will be responsible for providing high-quality nursing care to our short-term rehab and long-term patients and will work closely with other healthcare professionals to ensure optimal patient outcomes.
Needs: Part-time or Full-time NIGHT (10:00 PM - 6:30 AM) shifts available.
Pay: Subject to licensure and dependent on experience.
Responsibilities
- Administer medications and treatments as prescribed by physicians.
- Monitor and record vital signs, ensuring any changes in patient condition are reported promptly.
- Conduct diagnostic evaluations to assess patient health status.
- Provide primary care services, including wound care and catheter management.
- Maintain accurate medical documentation for all patient interactions and treatments.
- Collaborate with healthcare teams for effective case management and patient care planning.
- Educate patients and their families on health maintenance and disease prevention.
Qualifications
- Valid Licensed Nurse (LVN or RN) license in California.
- Experience in skilled nursing is preferred.
- Excellent communication skills, both verbal and written.
