This role focuses on improving health outcomes by providing support, education, and linkages to resources. The CHW-Care Coordinator will address social determinants by connecting patients to needed services, navigating the healthcare system, accessing and providing adequate support resources.
Duties & Responsibilities:
- Point of contact for patients and community, assisting with care coordination and navigation of health and social service systems.
- Outreach to establish strong partnerships with families/patients, community organizations, healthcare providers, and local agencies.
- Partner with Project Manager in recruitment, conducting, and supporting culturally appropriate health education classes and promote healthy eating, chronic disease self-management and/or behavioral health.
- Conduct needs assessment of the community and clients to connect them with resources in partnership and collaboration with Social Workers ensure continuity of care.
- Collect data and document ( track and report) all engagement and health status improvements, referrals, and progress in accordance with the program.
- Attend meetings and ongoing training and professional development sessions to strengthen skills and stay current with best practices.
