Patient Care Coordinator

Essential Duties and Responsibilities

• Cordially and professionally meet with and provide after care direction/resources for patients.
• Promptly responds to calls/mail in a professional, courteous, and helpful manner.
• Maintain filing system for patient after care needs and possibilities.
• With the assistance of the clinical care team and the patient; formulate Aftercare Plans that will ensure best possible outcomes
• Completes the Referral and Intake Processes accurately as it to pertains to referrals to aftercare referral source
• Meet with business development representatives and referral sources to ensure PCC has resources across the country that meet patient needs
• Observes all confidentiality regulations.

Education and Experience

• Bachelor’s degree in psychology, social work, or related field and or the educational and experience equivelent.
• 2 years’ experience in a mental health/healthcare field preferred.
