Valley Hospital is seeking a dynamic and talented Discharge Planner to join our team!
Schedule: FT (Tuesday - Saturday 8-5)
Job Description:
The Discharge Planner works with the Utilization Review department on certification and recertification of insurance benefits throughout the patient’s stay, and assists the treatment team in understanding the barriers to discharge planning. The Discharge Planner is responsible for having a thorough understanding of the patient’s treatment through communication with the treatment team. The Discharge Planner advocates for the patient’s access to services during treatment team meetings and through individual physician contact. The Discharge Planner will carry a caseload, will meet with patients daily, be responsible for completing referrals, family sessions, staffing’s, and coordinating with outpatient providers.
Valley Hospital is centrally located with easy access to many areas of greater Phoenix. We are between North 35th and 36th Streets with East Earl to our north and East Thomas to our south.

Qualifications
Master’s degree from an accredited college or university in social work, mental health, nursing, or related degree and two (2) years of behavioral health experience required.
Experience:
A minimum of two (2) years’ experience in a healthcare setting or managed care company, with experience in patient assessment, treatment planning, utilization management, and/or case management.
Licensure: L.M.S.W., LAC, LCSW, LPC, LISAC or other healthcare related licensure required.
