Overview
The care coordinator assists patients to arrange follow-up care with primary care physicians and specialists as needed and assists the case manager with discharge planning activities, such as transportation arrangements, making referrals, obtaining authorization for services, and delivering patient notices.
Qualifications
Required Education: High School Diploma or GED Preferred Education: Associates degree Necessary Skills: -Excellent interpersonal skills with effective verbal and written communication skills in a fast-paced setting -Detail oriented and above average organizations skills with the ability to multi-task and prioritize -Knowledge of community and outpatient agencies and resources -Strong personal computer skills (i.e. Outlook, Work, Excel) and good typing skills (20 wpm) Required Experience: 1-2 years clerical, secretarial, or customer service experience or the equivalent. Preferred Experience: Hospital setting
