General Purpose of the Job

This RN provides broad clinical support across UCP’s residential programs. The nurse strengthens regulatory compliance, supports continuity of care, and assists with coverage needs across the agency. This role requires strong OPWDD experience, sound clinical judgment, and the ability to step into varied environments without losing accuracy or accountability. 

 

Essential Duties and Responsibilities include the following but are not limited to (Other duties may be assigned):

 

Clinical and Compliance Support 

  • Conduct scheduled and unscheduled chart reviews and audits for all residential sites to ensure compliance with OPWDD and agency standards. 
  • Identify documentation gaps and follow up with residence nurses and supervisors to correct issues promptly. 
  • Assist with the creation, revision, and training of PONS for individuals across programs. 
  • Reinforce and support implementation of nursing policies and procedures, escalating concerns when required. 
  • Monitor and support utilization of electronic MAR and health records to ensure regulatory compliance.

Medication Management 

  • Complete AMAP pourings as assigned and ensure medications are prepared accurately and in compliance with agency protocols. 
  • Provide coaching to AMAP staff when patterns of medication errors or knowledge gaps are identified. 

Coverage and Support 

  • Provide RN coverage for residences with vacant nursing roles, maintaining continuity of care, medical follow-up, and required documentation. 
  • Support residential teams during transitions, complex medical events, or surveys and survey preparation. 

On-Call Responsibilities 

  • Participate in the nursing on-call rotation, providing timely and appropriate guidance to DSPs, supervisors, and residence nurses. 
  • Document all calls in accordance with agency standards and ensure follow-up occurs within the required timeframe. 

Training and Staff Development 

  • Deliver training and coaching to DSPs, AMAP personnel, and residential leadership as assigned. 
  • Support corrective action and re-training efforts following incidents, audits, or survey findings. 

Other Duties 

  • Collaborate with interdisciplinary teams to support resident care. 
  • Attend required meetings, including Nursing, QA, and administrative reviews. 
  • Complete additional tasks assigned by the Nursing Leadership Team. 

 

When Providing Coverage for Vacant Nursing Roles, Essential Duties and Responsibilities include the following: Other duties may be assigned. 

1. Ensure that all indicated medical appointments are scheduled and followed up appropriately. 

2. Ensure maintenance of all medical records per state and federal guidelines. 

3. Attend annual/interim meetings, weekly IDT meetings, and staff meetings. 

4. Participate in supervision and in-service training of direct care staff. 

5. Meet regularly with residence managers to discuss current health care concerns (minimum once weekly). 

6. Develop and implement program participant groups to discuss relevant health needs, i.e., nutrition, hygiene, medications, and medication policies if applicable. 

7. Ensure that individuals are living in a safe and hygienic manner. 

8. Review individual medications quarterly. 

9. Document medical and pharmacological services. 

10. Monitor health care services and recording systems weekly. 

11. Review all program participants' charts, i.e., med books, etc., weekly. 

12. Ensure all annual exams are completed in a timely manner, i.e., physical, dental, eye, etc. 

13. Insure there is an order for every medication filed on time. Insure the order, label, and medication charts match. Count the pills a minimum of weekly. 

14. Review and initial medication charts weekly to ensure accurate transcription of doctor's orders as needed. 

15. Document weights and B/P monthly. 

16. Include all medical appointments and illnesses for the month in monthly summary notes. Complete quarterly and annual evaluations. 

17. Give yearly recertification, including practicum, to each (AMAP) medication certified aide, and to certify staff in tube feeding, colostomy care and diabetes care as needed.

20. Implement medication guidance policy as needed. 

21. Adhere to all medical regulations governing ICF's and IRAs. 

22. Assist in accompanying individuals to medical appointments when necessary. 

Qualifications:

Qualified candidates for the Nursing Support Administrator position will have:

  • Current NYS Registered Nurse license 
  • 5 years of nursing experience preferred
  • 1 to 2 years related OPWDD experience and/or training with working knowledge of OPWDD regulations, AMAP guidelines, medication administration practices, PONS development, and documentation standards preferred
  • Valid NYS driver’s license 
  • Ability to participate in on-call rotation
