Responsibilities
The LVN Care Navigator will play a critical role in the patient experience including optimizing clinic flow, providing in and outreach to patients and working collaboratively with the care team to create a patient-centered environment. The LVN will do more computer and phone work versus patient care. This includes pre-visit planning, in-between-visit coordination, gathering data from different sources, seeking resources, clinical quality measures, and providing support for our population health management efforts in a primary care setting.
The LVN is responsible for sustaining an environment of intrinsic goodwill for the practice while providing optimum services to patients. The LVN provides licensed medical support to medical care providers as a vocational nurse for the department/clinical team to which it is assigned. The LVN is supervised by a licensed physician or registered nurse when performing duties within the vocational nurse’s scope of practice. The LVN works under the direction of clinic/department management for administrative purposes, including customer service, professional behavior & development, assignment & scheduling of work, and performance of job functions not requiring supervision by a LIP.
Required Qualifications
- California Licensed Vocational Nurse (LVN).
- Current American Heart Association (AHA) CPR certification or equivalent UCDH CPR Training.
- Two (2 ) years or greater of LVN experience in an ambulatory setting, is highly desired.
- Must provide proof of valid IV Therapy Certification and must pass the UCDH IV Therapy verification exam.
- Must pass UCDH Pharmacology and Venipuncture verification exams.
- Must provide documentation of graduation from an accredited school of vocational nursing, or completion of education and experience as a corpsman in the United States military.
- Proficiency in communication technologies (email, MyChart, Text etc.).
- Knowledge of clinic’s practices and protocols related to medical records, medical assisting, nursing support, and other business and office procedures.
- Knowledge of pertinent laws, regulations, and guidelines governing medical records; confidentiality, privacy, and provision to others, including HIPAA.
- Applicants are encouraged to upload their license and certification if required for the position.
Preferred Qualifications
- Experience with health IT systems and reports is desirable
- Knowledge of and/or proficiency in health behavior change or motivational interviewing preferred.
- Local knowledge about and connections to community health care and social welfare resources is desirable.
Special Requirements
- Must be willing to work variable days (including weekends), hours, and locations as required to meet the business needs of the practice.
- Travel may be required to any UC Davis Health Clinic.
- This position may be subject to a criminal background investigation, drug screen, Live Scan fingerprinting, medical evaluation clearance, and functional capacity assessment.
