Job Description
The RN-Care Transition is responsible for provision and management of patient care in a clinical area for specific age and patient populations. The RN-Care Transition delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with patient, significant others, and other health team members while maintaining standards of professional nursing. RN-Care Transition is responsible for accurately and efficiently triaging the telephone calls that are received from patients, their caregiver, relatives and other members of the healthcare team. This involves prioritizing patients’ health problems according to their urgency, educating/advising patients, scheduling appointments and making safe, effective clinical decisions under the direction of the patients’ physician.
Qualifications
Degree from an accredited school of professional nursing, and one (1) year of experience.
Preferred: BSN preferred. Pediatric experience is preferred.
License, Certification And/Or Registration
Current Alabama RN license. Current CPR certification or must be obtained within 30 days of employment.
