What are the responsibilities and job description for the SOCIAL WORKER-MSW REHAB | REHAB UNIT position at UAB Medical West Hospital?
The Social Worker completes psychosocial assessments and participates in treatment team meetings; provides individual and/or family conferences; screens patients on admission for transition of care; acts as a liaison with community resources/agencies: reports abuse and neglect to DHR; maintains a current list of community agencies/resources; assigns primary care physicians as needed; links patient with community resources i.e. nursing home placement, home healthcare, hospice, transportation, as well as plans, organizes, coordinates, and implements comprehensive social work services to assigned patients.
Master's degree in social work from an accredited college or university and a current Alabama Social Workers license. Minimum of six months acute care experience preferred. LGSW required, LCSW is a plus. Must possess Basic Life Support Certification by the American Heart Association or obtain with 30 days of hire.
