Job Summary
We are seeking a compassionate and dedicated Case Manager to join our team. The ideal candidate will play a vital role in supporting individuals through crisis intervention, addiction counseling, and behavioral health services. This role offers competitive hourly compensation, with the opportunity to earn up to $23 per hour contingent upon meeting established productivity and performance goals. The Case Manager will work closely with community agencies to ensure clients receive comprehensive support tailored to their needs, fostering positive outcomes and long-term stability.
Duties
- Conduct thorough assessments to identify clients' needs related to crisis management, addiction recovery, behavioral health, and social services.
- Develop and implement individualized care plans in collaboration with clients, families, and multidisciplinary teams.
- Provide crisis intervention and management to stabilize clients in urgent situations.
- Offer addiction counseling and motivational interviewing to support recovery goals.
- Coordinate services such as Section 8 housing assistance, Medicare programs, child welfare support, and workforce development resources.
- Monitor client progress through regular follow-ups and adjust care plans accordingly.
- Maintain detailed documentation of client interactions, service delivery, and progress reports.
- Collaborate with social workers, healthcare providers, childcare agencies, and community organizations to ensure comprehensive support.
- Supervise or mentor junior staff or interns as needed, fostering a team-oriented environment.
- Advocate for clients’ needs within various systems to promote access to resources and services.
Requirements
- Proven experience in crisis intervention, crisis management, or behavioral health settings.
- Knowledge of addiction counseling techniques and motivational interviewing skills.
- Background in social work or related fields; experience with child welfare, Section 8 housing programs, Medicare, or workforce development is highly desirable.
- Supervising or mentoring experience is preferred.
- Strong communication skills with the ability to engage clients effectively and empathetically.
- Ability to handle sensitive situations with professionalism and discretion.
- Experience working with diverse populations including children and families involved in child welfare programs.
- Relevant certifications or licenses related to social work or counseling are a plus but not mandatory.
