Dialysis Social Worker Role at US Renal Care

Key Responsibilities:
- Conduct thorough assessments to identify patient needs and develop effective care plans.
- Provide education and support to patients and their families to ensure optimal treatment outcomes.
- Ensure compliance with regulatory standards and company policies to maintain high-quality patient care.
Collaboration and Advocacy:
You will work closely with a dedicated team to coordinate patient care, identifying social agencies and resources to support patients and their families. Your advocacy skills will help facilitate access to community services, financial assistance, and other essential supports.
Clinical Processes and Quality Improvement:
As part of our interdisciplinary team, you will contribute to clinical processes that promote patient safety and minimize hospitalizations. You will participate in ongoing quality improvement activities, ensuring compliance with federal, state, and local regulations.
Requirements:
- Masters Degree in Social Work from an accredited institution (CSWE).
- Excellent communication and writing skills, with proficiency in English.
- Basic computer skills, including Microsoft Office (Word, Excel, Outlook).
- Ability to learn and proficiently use USRC clinical applications within 90 days.
- Prior experience in providing social services to dialysis patients is preferred.
