Summary

The Advanced Clinical Social Worker assigned to the Pain Management Clinic serves as an integral member of the interdisciplinary pain management team. The incumbent provides evidence-based psychosocial assessment - intervention - and care coordination for Veterans experiencing chronic pain - including associated psychosocial - behavioral health - and social determinants of health challenges.

Qualifications

Basic Requirements:

  • Education
  • Have a master's degree in social work from a school of social work fullyaccredited by the Council on Social Work Education (CSWE)
  • Graduates of schools ofsocial work that are in candidacy status do not meet this requirement until the School ofSocial Work is fully accredited
  • A doctoral degree in social work may not be substitutedfor the master's degree in social work
  • Verification of the degree can be made by going tohttp://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work
  • Licensure
  • Persons hired or reassigned to social worker positions in the GS-0185 seriesin VHA must be licensed or certified by a state to independently practice social work atthe master's degree level

  • One year of experience equivalent to the GS-11 grade level
  • Experience must demonstrate possession of advanced practice skills and judgment - demonstrating progressively more professional competency
  • Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship - or equivalent supervised professional experience

  • Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level - and must be able to provide supervision for licensure

  • In addition to the experience above - the candidate must demonstrate all of the following KSAs: (a)Knowledge of program coordination and administration which includes consultation - negotiation - and monitoring
  • (b)Knowledge and ability to write policies - procedures - and/or practice guidelines for the program
  • (c)Ability to supervise multidisciplinary staff assigned to the program
  • (d)Skill in organizing work - setting priorities - meeting multiple deadlines - andevaluating assigned program area(s)
  • (e)Ability to provide training - orientation - and guidance within clinical practice

Duties

  • Duties to include but not limited to: Clinical Responsibilities: Conduct advanced - psychosocial and behavioral health assessments for Veterans with complex chronic pain syndromes
  • Provide specialized evidence-based psychotherapies for chronic pain - including CBT-CP - CBT-I - ACT - MI -and behavioral activation approaches
  • Conduct suicide-specific evaluations including C-SSRS - CSRE - and safety planning interventions
  • Deliver complex care planning
  • Provide clinical case consultation to Behavioral Health - Primary Care - Pain Medicine - Physical Therapy -Whole Health - PCMHI - and community partners
  • Utilize advanced clinical decision-making to determine level of care - referral needs - care coordination intensity - and clinical risk mitigation strategies
  • Implement interdisciplinary treatment pathways for high-risk or high-utilization Veterans with chronic pain
  • Use telehealth-specific clinical competencies
  • Provide family or caregiver interventions to enhance understanding of chronic pain - reduce accommodation behaviors - and support engagement in evidence-based treatment modalities
  • Administrative Responsibilities: Assist with clinic workflow optimization - population-level management strategies - and development ofchronic pain care algorithms within the Pain Clinic
  • Leads or co-leads program evaluation efforts
  • Participate in facility and VISN chronic pain initiatives
  • Develops educational materials and trainings for interdisciplinary staff
  • Serves as a Pain Clinic subject matter expert on psychosocial factors influencing opioid risk mitigation -behavioral health relapse prevention - and treatment adherence
  • Contributes to policy/procedure development
  • Behavioral Health Coordination & Interdisciplinary Care: Serve as a primary behavioral health liaison for the Pain Clinic
  • Participate in interdisciplinary pain huddles
  • Coordinate care for Veterans with co-occurring mental health conditions
  • Manage complex transitions of care
  • Engage in shared treatment planning with Behavioral Health and Pain teams
  • Conduct behavioral risk assessments related to maladaptive pain responses
  • Provide consultation on opioid tapering support
  • Lead or participate in interdisciplinary case conferences for high-risk Veterans with chronic pain
  • Ensure continuity of care for Veterans with frequent hospitalizations or ED visits related to pain
  • Academic / Teaching / Training: Provide training to interdisciplinary trainees and new staff
  • Serve as a clinical preceptor for social work trainees
  • Other Significant Responsibilities: Participates in regional or national VHA pain practice groups - workgroups - or communities of practice
  • Contribute to development of Veteran self-management resources - psychoeducational tools - andgroup curricula
  • Assist in grant proposals - pilot initiatives - or quality innovations related to pain management orbehavioral health integration
