Duties

- Functioning within a Patient Aligned Care Team (PACT) - working collaboratively with the other PACT members - including the RN Care Coordinator - the LPN - and the Medical Service Assistant
- Works with extended PACT members - including pharmacists - mental health professionals - social workers - and dietitians
- Providing effective care to patients using sound judgement in assessing - planning - implementing - documenting - and evaluating patient care for assigned patients
- Performing history and physical examinations - ordering appropriate lab and other tests - evaluating test results - establishing diagnoses - formulating - and implementing treatment plans - and modifying treatment plans when appropriate
- Managing prescriptions for assigned patients - including controlled substances if warranted
- Working effectively and making appropriate requests for assistance from the collaborating physician as appropriate
- Assisting - as requested - with clinic procedures
- Answering patient questions regarding consult placement - follow-up and how to get patients seen
- Interacting with consultants as needed for successful referrals
- Referencing quality improvement information and organizes patient care needs to assure safe - efficient - and cost-effective care
- Serving as clinic level patient advocate
- Acting as the focal point of the designated outpatient clinic by being the contact person for all other consultant-specialty services regarding patient care
- The PA will ensure that the care is coordinated and communicated to the outpatient

- Relocation/Recruitment Incentives: Not Authorized Contract Buy Out Program: This position may be eligible for the Physician / Provider Contract Buy Out Program (CBOP) - which authorizes VHA to buy out service contracts (Locum agreements - loan repayments - sign-on bonus contracts - Physician-owned practices) in exchange for employment at certain rural or highly rural facilities for a period of obligated service (minimum 4 years)
