Home Based Primary Care Nurse Practitioner--Eureka VA Outpatient Clinic

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

• **This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until July 10 - 2026
• The initial cut-off date for referral of eligible applications will be May 25 - 2026 with subsequent cut-off dates every 2 weeks
• Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available
• NOTE: This position is based out of the EUREKA VA Outpatient Clinic - duties entail traveling to the homes of Veterans living in the surrounding area
• The Home Based Primary Care Nurse Practitioner (HBPCNP) at the Eureka VA Outpatient Clinic provides comprehensive - patient-centered primary care to a diverse Veteran population in an outpatient setting
• Working within the VA's Patient Aligned Care Team (PACT) model - the PCNP delivers high-quality - evidence-based care focused on disease prevention - health promotion - chronic disease management - and coordination of services across the continuum of care
• Given the rural and geographically isolated setting of Eureka - the HBPCNP plays a critical role in ensuring access to timely - continuous primary care services
• The position requires strong clinical judgment - adaptability - and a commitment to serving Veterans in an underserved region while upholding VA standards of care - performance metrics - and regulatory requirements
• The incumbent independently manages a panel of patients with acute and chronic health conditions - with an emphasis on health promotion and disease prevention - using evidence-based practice in the home of Veterans
• A valid Driver's License and insurance are required
• Major duties and responsibilities of the position: Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with the interdisciplinary team
• Orders and interprets labs - tests - and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
