Duties
- The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) in Home Based Primary Care (HBPC) practices in Extended Care and Palliative Medicine Service to provide a full range of health care services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
- The NP provides advanced clinical practice - consultation - and management
- The major duties and responsibilities include - but are not limited to: Independently manages a panel of patients with acute and chronic medical - social - and behavioral conditions - with an emphasis on caring for older - frail - homebound patients
- Visits patients in their homes in the community
- Assesses - identifies - and diagnoses patients' general medical problems and defines the medical management
- Develops and implements an individualized treatment plan to achieve optimal patient health outcomes
- Demonstrates leadership and works collaboratively with HBPC interdisciplinary team members
- All other duties as assigned
