Duties
The Geriatrics and Extended Care (GEC) Advanced Practice Registered Nurse (APRN) practices in the Community Living Center (CLC) to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission. The APRN provides advanced clinical practice - consultation - and management.
Duties
- The Advanced Practice Registered Nurse - Geriatrics and Extended Care Community Living Center provides care and counseling of persons in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering - assessment - appropriate nursing judgment - and evaluation of human responses to actual or potential health problems through such services as case finding - health teaching - health counseling
- provision of care supportive to or restorative of life and well-being
- and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider
- The nurse's primary commitment is to the patient - whether an individual - family - group - or community (ANA Code of Ethics for Nurses)
- Major duties include - but not limited to: Independently manages a complex patient panel with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Making clinical decisions with minimal physician oversight
- Initiate quality improvement projects based on clinical data analysis and clinical needs
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with the interdisciplinary team
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
