Duties
The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Gastroenterology (GI) within Specialty Medicine Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice - consultation - and management.
Duties
- The NP practices to the full extent of their education - training - and certification
- Core functions are outlined in the clinical privileges that correspond with this position
- The NP has prescriptive authority for a panel of patients or program as indicated by their position
- The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
- Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with the interdisciplinary team
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

- The NP will also function in concert with other GI Medical Staff and Gastroenterology (GI) Nurse practitioners to provide care for veterans with GI specialty care needs
- The NP will assess - plan - implement - coordinate - monitor - and evaluate options and services to meet the individual health needs - using excellent communication and available resources to promote a seamless health care experience with high quality and cost-effective outcomes
- The GI NP is responsible to the Chief of GI and works collaboratively with the GI Attendings - GI Fellows - other GI Nurse Practitioners - other GI personnel - and other services and disciplines
- The GI NP is expected to work with minimal direction and demonstrate excellent clinic assessment and intervention skills - critical thinking skills - excellent interpersonal skills - and effective communication
- The GI NP role implementation will require advanced clinical skills including patient identification

- coordination
- monitoring and evaluation of care
- data collection and analysis
- and documentation of outcomes - including cost - quality and client status
- While the job is based in Nashville some of the CQI functions and coordination of care will involve patients from both campuses and CBOCs and will be done remotely from the Nashville campus
- All of the direct patient care activities (clinics - etc.) will take place at the Nashville campus
