Duties
- The Neurology Nurse Practitioner serves as a key member of the Neurology team - providing advanced practice care to patients with neurological conditions
- The APN is responsible for managing the day-to-day neurology needs of patients
- The APN will work in collaboration with the interdisciplinary team to deliver high-quality care and optimize patient outcomes
- Major duties and responsibilities of the position: Independently manage a panel of patients with both acute and chronic neurological conditions - emphasizing health promotion and disease prevention through evidence-based practices
- Develop and implement individualized care plans to optimize population health and chronic disease management
- Prescribe pharmacologic and non-pharmacologic treatment interventions in accordance with VA protocols
- Initiate referrals and consults to neurology sub-specialties and other relevant services as needed Demonstrate leadership and collaborate actively with interdisciplinary neurology care teams
- Provide patient and family education on diagnosis - treatment options - self-management - and lifestyle adjustments
