Job Summary
We are seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will have a strong background in providing therapeutic services to individuals across various settings, including pediatrics, geriatrics, and acute care. This role involves assessing patient needs, developing personalized care plans, and implementing therapeutic interventions to enhance the quality of life for patients with diverse needs, including those with disabilities.
Responsibilities
- Conduct comprehensive assessments of patients' functional abilities and limitations.
- Develop and implement individualized treatment plans aimed at improving daily living skills (ADLs).
- Collaborate with multidisciplinary teams to ensure effective discharge planning and continuity of care.
- Provide therapy services in various settings including inpatient and outpatient.
- Work closely with patients to facilitate rehabilitation through therapeutic exercises and activities.
- Document patient progress and modify treatment plans as necessary based on ongoing evaluations.
- Educate patients and families about treatment goals, techniques, and home exercise programs.
- Maintain knowledge of medical terminology, anatomy, physiology, and relevant occupational health practices.
- Participate in case management activities to coordinate care across different healthcare providers.
