Core Responsibilities:
- Aiding and providing in-home support to meet individuals goals
- Assist members with activities of daily living (ADL's)
- Follow care plans and establish safety protocols
- Assist with medication (if needed)
- Promote inclusion, choice, and self-advocacy
- Accurately complete data sheets, time sheets and required documentation
- Maintain confidentiality and comply with HIPPA and agency policies and procedures
- Follow all agency policies and procedures, state regulations and HCBS requirements
- Participate in required training, certifications and ongoing education on time
Respite (RSP)
The qualified person will supervise and care for Tyler's Place Az members physical, mental and/or person caring for that member.
Attendant Care (ATC)
The qualified person will assist Tyler's Place Az member in creating/maintaining a safe, clean-living environment which may/may not include personal cleanliness. Provider will also assist members to engage in community outings and/ or activities and to help develop and maintain a daily schedule and/or daily plan of care.
Habilitation (HAH)
The qualified person will assist Tyler's Place Az members in a variety of interventions based on member's specific DDD goals to help them become more independent by improving their daily life and to be a valued member of his/her community. Interventions may/may not include Habilitation therapies, behavior interventions and/ or sensory motor implantations.

- Ability to stand, walk, bend, and assist members as needed

Qualifications:
- Level 1 Fingerprint Clearance Card
- Pass Central Registry Background Check
- Valid Az Driver's License (to transport members)
- Valid Driver's License (non-transport)
- Driver's Motor Vehicle Report (MVR)
- CPR & First Aid Certifications
- Article 9
- DCW ( Direct Care Worker) Certificate - if applicable
- 18 years of age or older
- 21 years of age or older to transport
Job Types: Full-time, Part-time
Shift Monday and Thursday 5:30am-7am and 2:30PM-3:30PM
