Job Summary

We are seeking a dependable, organized, and compassionate Program Coordinator/Supervisor to oversee the day-to-day operations of our 245D-licensed residential program.

This position plays an important role in ensuring that the individual we support receives consistent, safe, respectful, and person-centered care. The Program Coordinator/Supervisor will work closely with Direct Support Professionals (DSPs), supervisors, guardians/family members, case managers, healthcare providers, and other members of the individual's support team.

The ideal candidate is someone who is a strong communicator, comfortable taking initiative, and able to lead a team while maintaining a calm and supportive environment. This role requires attention to detail, good judgment, and a commitment to following the individual's care plan and all applicable program requirements.

Responsibilities

  • Oversee the daily operations of the 245D-licensed residential program.
  • Provide leadership, direction, and support to Direct Support Professionals and other staff.
  • Help ensure staff provide consistent, respectful, and person-centered care.
  • Coordinate staff schedules, coverage, and daily responsibilities.
  • Monitor staff performance and provide coaching, guidance, and feedback as needed.
  • Assist with onboarding and training new employees and ensure required training is completed.
  • Ensure staff follow the individual's service plan, care plan, behavioral support strategies, routines, and agency policies.
  • Maintain accurate and timely documentation, including progress notes, daily documentation, medication records, incident reports, and other required records.
  • Review documentation regularly for accuracy, completeness, and consistency.
  • Monitor the individual's health, safety, behavioral needs, and overall well-being.
  • Communicate concerns or significant changes promptly to the appropriate supervisor, guardian, case manager, or healthcare professional.
  • Coordinate appointments, transportation, activities, and other services as needed.
  • Communicate and collaborate with guardians/family members, case managers, healthcare providers, and other members of the individual's support team.
  • Help maintain compliance with 245D requirements, licensing standards, and agency policies.
  • Assist with developing and maintaining a safe, clean, and supportive residential environment.
  • Help address staff and program concerns and work toward effective solutions.
  • Participate in meetings related to the individual's services and care.
  • Support implementation of individual goals and promote independence, dignity, choice, and community involvement.
  • Respond appropriately to emergencies and ensure required follow-up documentation is completed.
  • Maintain confidentiality and protect the individual's privacy and rights.
  • Provide direct support when needed to maintain appropriate staffing and continuity of care.

Pay: $26.00 - $30.00 per hour
