Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation to develop tailored care plans.
- Administer medications and treatments such as IV infusions, injections, wound suturing, and catheter care following strict aseptic techniques.
- Perform specialized procedures including wound management, ostomy care, tube feeding, and ventilator management when necessary.
- Utilize electronic health record (EHR) systems like Epic or Cerner to document patient encounters accurately and maintain detailed medical records in compliance with HIPAA regulations.
- Educate patients and their families on disease management, medication administration, nutrition, and safety protocols to promote health literacy.
- Coordinate with interdisciplinary teams for discharge planning, case management, and referrals to other healthcare providers or community resources.
- Respond promptly to patient triage calls and urgent situations such as acute pain episodes or infectious disease concerns with appropriate interventions.
Skills
- Proven experience working with diverse populations including seniors in geriatrics, memory care units, behavioral health settings, or individuals with developmental disabilities.
- Strong knowledge of medical terminology, ICD coding (ICD-9/10), and diagnostic evaluation techniques.
- Skilled in performing invasive procedures such as phlebotomy, venipuncture, catheterization, and laboratory specimen collection/processing.
- Familiarity with EMR/EHR systems like Athenahealth or eClinicalWorks for seamless documentation and patient tracking.
- Critical care experience from ICU or trauma centers enhances your ability to handle complex cases involving acute pain management or trauma medicine.
- Ability to work independently while maintaining high standards of infection control and aseptic technique.
- Excellent communication skills for patient education, documentation accuracy, and collaboration with healthcare teams.
