Job Summary
We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across diverse needs. This vital role involves assisting individuals with daily activities, ensuring their comfort, safety, and well-being. Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote hygiene and comfort.
- Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report any abnormalities promptly.
- Support medication management by administering prescribed medications following medical instructions and maintaining accurate medical documentation.
- Provide companionship and emotional support to reduce feelings of loneliness or isolation, especially for clients with dementia or Alzheimer's disease.
- Assist with mobility and transfers using patient transfer methods and equipment like Hoyer lifts or gait belts to ensure safety during movement.
- Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living environment for clients.
- Maintain strict adherence to home healthcare regulations, HIPAA privacy standards, and confidentiality protocols while documenting care activities in Electronic Health Record (EHR) systems.
Skills
- Proven experience working with individuals with developmental disabilities, geriatrics, or in hospice care settings.
- Strong knowledge of medical terminology, medication administration procedures, and basic life support (BLS) including CPR.
- Skilled in patient observation techniques and recognizing signs of health deterioration or emergencies requiring immediate attention.
- Familiarity with home health agency operations, EMR/EHR systems for accurate medical records management, and home & community care practices.
- Excellent communication skills for effective client interaction and collaboration with healthcare teams; compassionate caregiving approach.
- Ability to perform heavy lifting safely when required and operate mobility aids such as Hoyer lifts or patient transfer devices.
- Knowledge of home health care assistance regulations, dementia care strategies, end-of-life support practices, and hospice protocols.
