Responsibilities
- Conduct comprehensive patient assessments to identify needs across medical, social, and behavioral health domains.
- Develop individualized care plans that align with clinical guidelines and patient preferences.
- Coordinate services with multidisciplinary teams including social workers, physicians, nurses, and community resources.
- Manage utilization review and utilization management processes to ensure appropriate care levels and resource use.
- Facilitate discharge planning by arranging follow-up care, home health services, or community support programs.
- Maintain accurate documentation within Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems in compliance with HIPAA regulations.
- Communicate effectively with patients and their families, providing education on treatment options and care plans.
Qualifications
- Proven experience in critical care, hospital settings, or emergency medicine environments such as ICU, PICU (Pediatric Intensive Care Unit), or Level I/II trauma centers.
- Strong knowledge of medical terminology, CPT coding, ICD-9/ICD-10 coding systems, and managed care processes.
- Background in nursing, social work, behavioral health, or related healthcare fields with relevant licensure or certification preferred.
- Familiarity with EMR/EHR systems and proficiency in Microsoft Office applications for documentation and reporting.
- Excellent client communication skills with the ability to work effectively with diverse populations from various cultural backgrounds.
- Experience in discharge planning, utilization review, and case management within hospital or hospice care settings highly desirable.
- Knowledge of HIPAA regulations and health insurance processes to ensure compliance and confidentiality.
- Bilingual abilities are a plus to serve a broader patient demographic effectively.
