Job Summary

We are seeking a highly qualified and compassionate Professional to join our healthcare team. The ideal candidate will possess extensive experience in hospital settings, including trauma centers, ICU, pediatrics, and behavioral health. This role involves providing comprehensive patient assessment, care coordination, discharge planning, and clinical case management. The Qualified Professional will work closely with multidisciplinary teams to deliver exceptional patient service while ensuring compliance with healthcare regulations and maintaining accurate medical records. Candidates with a background in social work, mental health counseling, addiction counseling, or related fields are encouraged to apply. Duties

  • Conduct thorough patient assessments, including physiological evaluations and diagnostic evaluations.
  • Develop and implement individualized care plans in collaboration with medical teams and patients’ families.
  • Manage electronic health records (EHR/EHR) systems accurately and efficiently, ensuring HIPAA compliance.
  • Coordinate community resources and social services to support patient needs post-discharge or during outpatient care.
  • Facilitate discharge planning by assessing patient needs and arranging appropriate post-acute or outpatient services.
  • Provide behavioral health support, including mental health counseling and addiction counseling as needed.
  • Prioritize tasks effectively within fast-paced hospital environments such as Level I Trauma Centers, PICUs, or emergency departments.
  • Communicate clearly with patients, families, and healthcare providers to ensure understanding of treatment plans and care options.
  • Assist in case management activities, including utilization management and client communication for diverse populations.
  • Support inpatient, outpatient, clinic, or post-acute care settings by delivering patient-centered care aligned with clinical protocols.

Requirements

  • Proven experience working in hospital environments such as Level I Trauma Centers, ICU, PICU, or emergency medicine settings.
  • Strong background in acute care, inpatient services, outpatient clinics, or post-acute care programs.
  • Demonstrated expertise in medical terminology, physiology knowledge, and diagnostic evaluation techniques.
  • Proficiency with EMR/EHR systems and electronic health records management; familiarity with HIPAA regulations is essential.
  • Valid certification in Basic Life Support (BLS) and CPR; additional certifications such as Mental Health Counseling or Addiction Counseling are advantageous.
  • Experience working with children, individuals with disabilities or substance use disorders, and culturally diverse populations.
  • Excellent task prioritization skills combined with strong client communication abilities.
  • Knowledge of community resource coordination within health services social work frameworks is preferred.
  • Ability to work effectively within multidisciplinary teams across inpatient and outpatient settings while maintaining high standards of patient service and confidentiality.
