Overview: As a Turning Leaf Case Manager, you will be expected to function as an active Team Member to provide case management services and treatment based services to consumers with mental illness, developmental disability and/or traumatic brain injury. Duties include one on one interaction with consumers and related activities including coordinating relevant consumer specific supports, services, housing, social networks, appointments, PCP planning, utilizing natural/community supports, maximizing income/benefits and plans of service.
Provide individual therapy, group therapy, skill building programming and case management services for consumers of Turning Leaf. Duties include face-to-face contact and related activities including coordinating relevant consumer specific supports, services, housing, social networks, appointments, PCP planning, utilizing natural/community supports, maximizing income/benefits and plans of service. Monitors the implementation and effectiveness of individual treatment and behavior plans. Functions as a liaison to various state, county, and private agencies involved with Turning Leaf or its consumers. Provides communication across, within and between all stakeholders and community resources providing services to the consumer.
Qualifications: Bachelor's degree in a human services field preferred (i.e. psychology, social work, etc.). Master degree and/or preferred credentials include State of Michigan LMSW, LLMSW, LP, LLP, LPC, LLPC, LMFT, LLMCT. Certified Alcohol & Drug Counseling (CAADC/CADC) highly preferred. Priority consideration will be given to candidates who possess residential behavioral health experience.
Job Requirements- Essential:
Minimum of a Master’s degree in Social Work, Psychology or Counseling along with appropriate State of Michigan clinical licensure and at least three years successful clinical experience working with the SPMI, DD and TBI population.
Contingencies: All opportunities at Turning Leaf are contingent upon successful completion of a post-offer physical examination, pre-hire tuberculosis testing, criminal background clearances, as well as degree and/or license verification. A valid Michigan Driver’s License is required at time of employment and must be maintained throughout employment.
Salary : $40,000 - $50,000 annually; subject to negotiation based on education and experience.
