JBBS Case Manager

About the Role
TK Health Clinics is seeking a JBBS Case Manager to support the Colorado Division of Behavioral Health – Jail Based Behavioral Health Services (JBBS) Program. This role plays a critical part in providing screening, assessment, coordination, and continuity of care for individuals with substance use disorders and co-occurring mental health conditions while they are incarcerated.
The JBBS program partners with county sheriffs to ensure individuals receive appropriate behavioral health services during incarceration and are connected with community-based care upon release.
This position is ideal for a detail-oriented, mission-driven professional who is comfortable working in a correctional environment and collaborating with multidisciplinary teams.
Key Responsibilities

Organize and manage referrals for newly admitted inmates

Interview referred inmates to determine JBBS eligibility and interest

Administer and score screening and assessment tools, including:

ASUS-R

HELPS

PCL-C

Identify client risks and needs and make appropriate referrals to Mental Health (MH) or Substance Use Disorder (SUD) clinicians

Coordinate referrals and scheduling for psychiatric medication evaluations and reviews

Facilitate psychoeducational groups such as Life Skills, MRT, or other recommended programming
-
Coordinate with clinical staff, jail staff, and community partners to schedule telehealth appointments

Respond to inmate requests and kites in a timely and professional manner

Provide inmates with self-help and therapeutic materials

Coordinate referrals with outside community agencies for:

Housing
-
Outpatient mental health services
-
Substance abuse treatment
-

Prepare treatment evaluation reports, treatment plans, progress letters, and verbal updates for legal entities as requested (Courts, Probation/Parole, Attorneys, Social Services, Competency Programs, etc.)

Collect and enter clinical and program data into internal databases and the state-required JBBS tracking system (CiviCore)

Track screened, admitted, and discharged clients

Complete post-release follow-ups as required by the JBBS program

Maintain confidentiality and comply with HIPAA regulations

Maintain accurate records and case documentation

Respond to phone calls and emails and utilize standard office equipment

Qualifications

Experience in behavioral health, case management, or social services (correctional experience preferred but not required)

Ability to pass a thorough background check
